We are currently sending text messages to patients with a link attached for a questionnaire to be completed. Thank you for your co-operation.



 Patients will still need to wear a mask, socially distance and use hand sanitiser when attending the Surgery and our patient-facing staff will continue to wear PPE.


 


We do understand that it can be frustrating but we know that when the patients come to us they may be sick and vulnerable, and some cannot receive the Covid vaccine due to medical reasons, so we feel it would be wrong to not continue with these small precautions that can help to protect the patients and staff.

All patients wishing to register must provide ID and proof of address to confirm residence within our practice area. A map of the practice area can be found on the above tab.

All newly registered patients will be asked to complete a questionnaire and to have a check-up with a member of the nursing staff within one month of registering with us. This helps to give us some background information about you that will help us in looking after you.

Registration forms and questionnaires are available to download from the above "Registering Online" tab.

You are able to attend the doctor once you have registered.
